I need to have a custom filename assigned to the documents being printed.  I would like to have 4 fields that the user can select data for the field and two other fields that they can enter typed information. I need the ability to select the file name delimiter, under score or hyphen.

Is anyone able to assist me on this item.  Is there anyone that can create a custom file naming screen that prompts the user to select the items for the four fields and enter the data for the two additional field.
